Conditions of participation

Our specialised programme is aimed at patients with advanced heart failure (NYHA class II or III with an ejection fraction of < 40%).

Whether or not you are eligible for this specialised programme depends, among other things, on whether

• you have a heart-supporting device (ICD or CRT device) or were treated in hospital for acute worsening of your heart failure in the last year,
• your heart failure is being treated in accordance with the recommended guidelines, and
• you are able to carry out the data measurement and transmission and therapy adjustments independently.

Procedure

We will provide you with a scale, a blood pressure monitor, a mobile ECG and a tablet computer. With the help of this network of devices, you can easily transmit your measurements to us wirelessly and from the comfort of your own home – every day.

Our staff will check these readings and, if necessary, contact you or your doctor immediately.
