
The Transsectoral Heart Failure Network Berlin Southwest Brandenburg
In the Transsectoral Heart Failure Network Berlin Southwest Brandenburg patients with chronic heart failure receive comprehensive care together with cardiological practices and GPs in the region and receive valuable information in free training courses on how to deal with their disease independently in everyday life.
The network was founded in 2017 by the Clinic for Cardiology, Angiology and Intensive Care Medicine at the Benjamin Franklin Campus together with specialist cardiological practices, GP practices and the Sport-Gesundheitspark Berlin. The network partners work closely together and offer, among other things, training for patients and their relatives to ensure that heart failure patients receive optimal care and are strengthened in their self-management.
Objective and network partners
The network enables customised therapy with the aim of reducing hospital stays. Fixed contact persons ensure that patients receive care across all sectors (i.e. outpatient treatment, inpatient treatment, treatment in a cardiological or general medical practice). This reduces the risk of duplicate prescriptions or medication errors and improves the prognosis and quality of life of those affected.
The Transsectoral Heart Failure Network Berlin Southwest Brandenburg currently includes a total of 19 cardiological and general medical practices, the Sport-Gesundheitspark Berlin, the Reha-Zentrum Seehof and other clinics
