Patient management at the DHZC

Patient management at the DHZC is responsible for all elective (i.e. planned) cardiological patient admissions, transfers and discharges. The patient managers at the DHZC are your main point of contact when it comes to scheduling and preparing for your planned cardiological stay at the DHZC. They ensure that the planning process runs as quickly and smoothly as possible for you. To this end, they will, with your consent, contact your treating GP and the hospital doctor at an early stage in order to obtain all the necessary information for the treatment process at the clinic.

Information on inpatient admission

The patient management staff will inform you of the date of your inpatient admission and provide you with all the necessary information about your planned procedure. Pre-admission scheduling up to 5 days before admission includes ECG, blood sampling, any outstanding diagnostic planning such as CT, MRI, X-ray, TTE, TEE and anaesthesia as well as a nursing history and medical information.

Care after inpatient admission

Once you have been admitted as an inpatient, the inpatient management team will take over. The patient manager will welcome you, explain the procedure to you, coordinate your examinations and take you to your room. Before or on the day of admission at the latest, the patient management team will evaluate how your care at home is guaranteed after discharge and, if necessary, ensure continuity of care.

Individual discharge planning

The therapeutic team, consisting of nursing staff, patient managers, social workers and doctors, will work with you - and, if necessary, your relatives - to plan your upcoming discharge or further care. Your individual needs will be assessed and the aftercare providers will be fully informed. The patient managers assume the management function and are your central contact persons on the ward.

Involvement of further experts

If necessary, the patient managers consult other experts and therapists such as wound managers, physiotherapists, nutritionists, psychologists, etc. They act as an interface between you as a patient, your relatives, the doctors, carers, therapists and social services.

Social services on discharge

The social services are actively involved in many discharges, particularly in advising on and organising rehabilitation measures, clarifying the assumption of costs by health, care or pension insurance and providing support with applications (degree of care, severe disability, aids, etc.).

Discharge home

If required, Social Services (SD) will organise an outpatient care service, care advisors, home help and also arrange the necessary provision of special products or aids by a home care company (e.g. medical supply store). If you need nursing support from relatives at home, the nursing team will instruct them if necessary, for example in changing dressings or administering injections correctly.

Discharge to another facility

The patient managers are your contacts for patients over the age of 70 who are undergoing early geriatric rehabilitation. They will advise you and have a good network at their disposal. Patient management also provides support with registration and appointments in rehabilitation clinics, nursing homes, hospices, residential communities and with registration for neuro-early rehabilitation phases B and C.

Discharge management at the DHZC

Legal framework and your consent

Discharge management at the DHZC is carried out in accordance with statutory provisions. We require your written consent to carry out discharge management, which is requested when the treatment contract is concluded. Without your consent, we cannot exchange the information necessary for safe further treatment.

Planning and organising your discharge

Your doctor will work with you to agree on a suitable discharge date and discharge destination. If you are transferred directly to a rehabilitation facility, an inpatient care facility or another hospital, we will organise transport.

For transport home, we can issue you with a taxi licence, which you must initially pay for yourself; reimbursement by your health insurance company is possible.

If you are unable to drive home independently, we will organise an ambulance to take you to your home, for which you will have to pay a statutory co-payment of 10 euros.

Medical care after discharge

We determine your need for medication, remedies and aids as well as home nursing care at an early stage and issue the necessary prescriptions. If necessary, the doctor can also issue a certificate of incapacity for work.

Prescriptions may only be issued if the patient's care after hospital treatment cannot be ensured in another, more economical way. Prescriptions and certificates may be issued for a period of up to 7 calendar days. Prescriptions must be honoured within 3 working days.

Further support services

When you are discharged, you will receive a doctor's letter and a medication plan. If you require a DVD of your examinations or a printout of findings, you can request these from Patient Management. You will also receive a discharge certificate from Patient Management when you are discharged.

If you are transferred directly to a follow-up treatment centre (AHB) or another facility providing further treatment, we will send you all the necessary information to ensure your care.
